Submit Permit Declaration for Licensed Contractors

3. Read the City Licensing Requirements Declaration

4. Residential Tenant Protection – Check the applicable box. If this address has Tenants in the dwelling unit (s) and you need more than 1 day to complete your work, you MUST obtain an approval of a satisfactory means and methods plan and you will need to comply with all its conditions PRIOR to starting construction.

5. Asbestos Removal Declaration – Check the applicable box *Note - If there is NO asbestos, please click on the middle box

6. Licensed Contractor’s Declaration – Check the applicable box. Enter your License Class and License Number

7. Worker's Compensation Declaration - Check the applicable box and Enter Workers' Compensation information.

8. Construction Lending Agency Declaration – Check the applicable box and Enter the lender information.

9. Final Declaration of Permit Holder – Enter your CA Driver’s License number and your Full Legal name.

10. Click Submit
